Responsibilities:

• Provide a full range of Physician Assistant services in primary and specialty medical care
• Deliver quality care for eligible beneficiaries in a Patient Centered Medical Home clinic
• Act as a patient advocate and promote desired health outcomes through education
• Collaborate with staff agencies to enhance patient care delivery
• Support research activities and participate in quality improvement initiatives

Qualifications:

• Completion of an accredited Physician Assistant program
• Current PA license and certification
• Experience in primary care and patient education
• Strong communication and collaboration skills
• Ability to work effectively in a team environment
