What You'll Do:

Provide Comprehensive Care: Conduct comprehensive assessments, develop and implement treatment plans, and monitor patient progress.
Educate and Support: Educate patients and their families about their condition, treatment options, and available resources.
Collaborate with the Care Team: Work closely with other healthcare professionals, including physicians, nurses, and social workers, to ensure coordinated and effective care.
Improve Patient Outcomes: Focus on improving patient outcomes, reducing hospitalizations, and enhancing the overall quality of life for individuals with chronic kidney disease.
What You'll Bring:

Strong Clinical Skills: Active and unrestricted Registered Nurse and Nurse Practitioner license with relevant board certification (ANCC/AANP).
Clinical Experience: 2+ years of direct patient care experience, including experience with chronic kidney disease.
Excellent Communication and Interpersonal Skills: Ability to effectively communicate with patients, families, and other healthcare professionals
Employment Type: Full-Time
Salary: $ 155,000.00 Per Year
