Job Description

This position is responsible for assisting in the development and implementation of clinical services for the Virtual Hospital - Hospital Care at Home program; obtain patient drug histories, provide patient education, recommend, implement, and monitor drug therapy regimens for patients, conduct pharmacokinetic monitoring and dosing for specific drugs, parenteral nutrition monitoring, conduct medication use evaluations and participate in quality improvement and cost-containment initiatives, and to assist in the clinical training of staff.

  • Consult with nursing and medical staff regarding prescriptions, orders, drug and pharmaceutical detail, patient reactions, errors, and complaints.
  • Conduct Drug Utilization Reviews and prepare Drug Review Criteria for medical staff review and approval.
  • Monitor drug therapy in collaboration with the medical staff.
  • Suggest alternative drug therapy as appropriate.
  • Manage drug therapy regimens according to medical staff-approved protocols.
  • Assist with medication reconciliation on admission and discharge to the Virtual Hospital - Hospital Care at Home to ensure smooth transitions and efficient timing and dosing of medications.
  • Participate in medical staff or hospital committees dealing with medication use.
  • Assist in the development and implementation of clinical pharmacy practices consistent with the Virtual Hospital needs and objectives.
  • Participate in virtual rounds with medical staff and assist in establishing individual patient drug therapy programs.
  • Conduct comprehensive profile reviews, pharmacokinetic monitoring and dosing, anticoagulation therapy monitoring and dosing, discharge counseling, renal dosing, parenteral nutrition monitoring and adjustments for patients in the Virtual Hospital.
  • Interview patients virtually to obtain drug histories.
  • Counsel patients virtually regarding proper drug dosage, proper administrations and possible side effects.
  • Assist in the development of staff educational and training programs within and relating to the Virtual Hospital - Hospital Care at Home program.
  • Participate in the training of staff.
  • Assist in the development and implementation of departmental analytical resources for drug analysis.
  • Participate in research programs and projects related to the utilization and effectiveness of clinical pharmacy practice.
  • Assist in the continued development of the central Medication Use Policy Service (AKA Drug Information).
  • Publish articles on Pharmacy practice in the P&T newsletter, other internal communications, and professional journals.
  • Process patient care orders using the electronic ordering system may be required in certain areas.
  • Works with leadership to ensure that patient care meets established quality standards, and complies with federal, state, and local regulations.
  • Reviews physician orders for technical and clinical accuracy and appropriateness. 
  • Maintains current knowledge of all institutional, state, and federal policies and regulations as it relates to Pharmacy and acts as a resource for department personnel in this area.
  • In conjunction with leadership, participates in orientation of staff regarding Conditions of Participation, third party coverage and other insurance coverage and documentation requirements.
  • Participates in accreditation survey readiness activities and the survey process.
  • Participates in organization sponsored staff education.
  • Oversees medication errors, event reporting, metrics for the Virtual Hospital. Sets targets and drives action plans to achieve desired results.
  • Participate in the Virtual Hospital - Hospital Care at Home QAPI Committee.
  • Participate in the Virtual Hospital - Hospital Care at Home Patient Safety Committee.
  • Monthly chart audits for compliance, validation of action items being carried out, and identifying opportunities for improvements.
  • Reviews, monitors, and evaluates clinical and administrative processes/workflows to ensure high-quality safe care.
  • Works collaboratively and communicates effectively with Hospital Care at Home Administrative Director and Medical Director, and other core support team members.
  • Partners with other business units within Post-Acute Care Services (PACS) ? Michigan Visiting Nurses, House Calls, Patient Monitoring at Home, SubAcute, Wheelchair Seating Service, HomeMed Specialty Pharmacy, PACS Triad, Providers, and other University of Michigan Health faculty and staff.
  • Collaborates with Adult Hospitals.
  • Takes responsibility for working knowledge of University of Michigan, Michigan Medicine and departmental policies and procedures, including the compliance program.
  • Completes mandatory competencies annually and as necessary.

  • A Doctor of Pharmacy degree or an equivalent combination of education and experience is necessary.
  • A Certificate of Residency from an accredited program or an equivalent combination of education and experience is necessary
  • Some experience in clinical pharmacy practice is necessary.
  • Some experience in didactic and clinical teaching, including undergraduate and postgraduate level courses, is necessary
  • Current licensure as a Registered Pharmacist by the State of Michigan is necessary.
  • Possession of a state-controlled substance license is necessary.

Physical Requirements
The employee will be able to perform the assigned duties and responsibilities with or without reasonable accommodations: ability to work in a sedentary position for long periods of time, ability to communicate/ exchange information, fine motor coordination/dexterity necessary for keyboarding or typing, and may require standing/walking, reaching, bending, twisting, stooping, moving about, lifting of up to 10 pounds, etc., with or without the use of assistive/adaptive devices.

  • Recent inpatient clinical hospital practice experience including participating in multi-disciplinary rounds, direct patient care and education, and product verification with a range of medications and patient populations.
  • Completion of an ASHP accredited PGY1 residency and/or BCPS certification.
  • Some experience in didactic and clinical teaching, including undergraduate and postgraduate level courses, is necessary.
  • LEAN Certification and / or 6 sigma green/black belt.
  • Excellent computer skills with working knowledge of Epic.
  • Knowledge of University of Michigan and Michigan Medicine policies, procedures, and practices.
